You have to turn the green paper over - I think it's in fine print on the back. It's in the bit where it asks things like: a. Have you now or ever been, involved with Nazi Germany? b. Are you entering the US for the purposes of Moral turpitude. c. Are you a member of terrorist organisation? If you've answered 'Yes' to any of these, please contact your nearest US embassy before attempting to enter the country. Course - if you're at 35,000 feet, calmly crammed into cattle class - about 3 hours out from landing at LAX - there's not many freely available US embassys.
