i would go with:
PERFORMER "Underworld"
TITLE "Pizza For Eggs"
FILE "audio.mp3" WAVE
TRACK 01 AUDIO
TITLE "Food A Ready"
INDEX 01 00:00:00
TRACK 02 AUDIO
TITLE "Back In The Fears"
INDEX 01 06:58:00
TRACK 03 AUDIO
TITLE "Vanilla Monkey"
INDEX 01 9:14:00
TRACK 04 AUDIO
TITLE "Flatz"
INDEX 01 13:42:00
TRACK 05 AUDIO
TITLE "Ancient Phat Farm Coat"
INDEX 01 14:16:00
TRACK 06 AUDIO
TITLE "Play Pig"
INDEX 01 22:22:00
Which brings in Back In The Fears at 6:58 not 7:05, brings in Flatz a second earlier and Play Pig 10 seconds earlier where Karl starts to say "Amnesia"(?). On the JAL to Tokyo EP, Food A Ready is 7:22 so you get an extended ending and the Ancient Phat Farm Coat is 8:05 vs 19:43 on the Japanese EP so its more than double the length.
To repeat the instructions I gave in the
LBT Split Thread: Open the cue file you just saved above in mp3directcut and then using the Save Split... option with "%N %D - %T" and number starting at 01, it created 01 Underworld - Food A Ready.mp3 etc. Unfortunately it doesnt create ID3v2 tags, only v1.1, but Winamp can copy to v2 and then I can add the same comments that were in the original mp3.
mp3directcut is here:

EDIT: for mp3 tag editing, I recommend
The Godfather which is a "postcardware" app. Its great at copying tags around and is also a great file renamer.
