Rowla promo, I believe. Supposedly pretty rare.

I got one, but it got bent terribly when it was shipped to me.
Mine's completely blank white labels, and says "UW Promo . 1 . A2" and "UW Promo . 1 . B2" in the runout grooves. Is that what yours looks like?
